Information Form
This form is to be used to obtain background information from the NAAR on the ancestry of your dog registered with
National American Alsatian Registry (NAAR).
As the owner of the dog, complete the OWNER SECTION of the form and then enter the dog’s NAME, REG #, BREED,,
COLOR, and SEX on the reverse side of this form. Submit the form to the breeder and have the breeder of the dog enter the
background information.
Background information for COLOR and REG. #s is optional. The breeder must record at least three (3) generations
on the dog. 4th generation information may also be included if it is available. The breeder must also complete the BREEDER
SECTION on the front side of the form, sign it, and have it notarized to verify that the background information is accurate
and true.
After the form is completed and mailed to our office, we will enter the background information into our records and print
out a Records Check Document for the dog.
